Since breaking into Hollywood as Bridget in "The Sisterhood of the Traveling Pants," Blake Lively has had a stunning style transformation. Over the years, her facial features have changed as well, leading many to speculate that she's had plastic surgery, and Dr. Samuel Golpanian doesn't disagree. "In her younger photos, she definitely has a thin upper lip, a wider nose than she does now. Her nose is less chunky so I think ... she had rhinoplasty with some tip work. She may have had some filler into the upper lip as well to plump it up," the double-board-certified Beverly Hills plastic surgeon exclusively tells Glam.

Dr. Golpanian notes that Lively didn't wear much makeup in her earlier acting days, so that may make a difference in her appearance now as well. However, he adds, "As you go from about 2009 and on, that's when I think she has had some of the nose work done. And as you keep progressing to 2019, her upper lip definitely looks more voluptuous, and she also has more prominent cheekbones." The plastic surgery expert thinks Lively has had filler or a fat transfer in that area, as well as possibly in her upper lip. "But that's usually when [celebrities] get a facelift. I don't think she got a facelift, though. I think the work on the nose and the lip is something that seems more obvious than anything," he tells us. The "It Ends With Us" actor's alterations are pretty subtle, and Blake Lively looks flawless even makeup-free, so she may just need a couple of simple procedures in the future to maintain her beauty.

A bit of maintenance never hurt anyone, and Blake Lively could keep up her youthful appearance with a couple of outpatient procedures, according to Dr. Samuel Golpanian. "I think maybe a brow lift, maybe as she gets older, because she does have a little bit of what's called hooding of the brow, so giving her a little bit of a brow lift can help that," he exclusively shares with Glam. The plastic surgeon adds, "Or maybe an upper blepharoplasty, a surgical procedure to rejuvenate the upper eyelids at some point — I don't think she needs it right now, but she might when she gets a little bit older."

In the meantime, Lively is using high-end beauty products to give herself that natural-looking glow. In 2019, the "Savages" star shared a glimpse of her collection on Instagram, which included makeup and skincare products from Dior, Caudalie, and La Mer. With her enviable stash (more on that in a bit) and Dr. Golpanian's expert suggestions, Lively may very well be one of Hollywood's famous older women who don't seem to age.

Our exclusive chat with Dr. Samuel Golpanian makes it clear that aesthetic procedures have definitely enhanced Blake Lively's look, and Dr. Gary Linkov agrees. The New York-based plastic surgeon also believes non-invasive treatments have played a major role in her transformation, especially when it comes to preserving that youthful glow. "At the age of 28, I'm seeing some signs of the start of Botox treatments to the common areas," Dr. Linkov shared in a YouTube video from September 2024, pointing to her forehead, glabella (the patch of skin between one's brows), and crow's feet. By starting young, he explains, she's avoided forming deep expression lines altogether. (For reference, she's 37 at the time of writing.)

But it's not just injectables. Dr. Linkov also points to Lively's skincare regimen as a major factor in her radiant appearance. He believes she's had treatments like laser resurfacing, microneedling, PRP, and regular facials to keep her complexion looking fresh and camera-ready. Interestingly, a little internet digging reveals he's right — Lively doesn't sleep on her skincare routine.

However, despite her A-list income, her skincare shelf includes some surprisingly affordable picks. While there's often debate between over-the-counter and medical-grade skincare products, the "Gossip Girl" alum seems to swear by a mix of both. According to Women's Health, Lively's go-to moisturizer is the CeraVe Facial Moisturizing Lotion SPF 50. Of course, she doesn't skip the luxe items, either. Lively is also a fan of La Mer's The Eye Concentrate, which retails for $275. With her balanced approach to beauty, Lively clearly has the forever-young routine on lock.
