RHOSLC Star Heather Gay's Face Has Totally Transformed (And Not Just From Ozempic)
Heather Gay's face has undergone quite the transformation over the years. And that transformation becomes clear as day when we compare a photo of Gay from 2012 with one from 2023. At nearly 49, "The Real Housewives of Salt Lake City" star appears to have more volume in her cheeks than she did at 38. Even her lips seem more plump in comparison to the throwback photo.
In a November 2025 interview with Bustle, Gay explained that she wasn't concerned with her looks when she was a single mother. However, once she found her financial footing, she thought: "I am going to try to profit from every single thing that's available to me. I can afford it, and it's part of my job now to look good." Her transformation began with Botox injections in her early 30s. As she entered her 40s, she started getting lip filler, had two nose jobs, one of which was a corrective procedure, and underwent a blepharoplasty.
She also got a tummy tuck and a breast augmentation as part of her "mommy makeover" surgery, which is the cosmetic procedure that Dr. Terry Dubrow from "Botched" exclusively told Glam he would never recommend. Gay also experimented with facial fillers during the early seasons of "RHOSLC," but ultimately stopped using them because they made her look completely different. By 2025, she was using a collagen stimulation procedure to get a similar effect to fillers. Over time, Gay also appeared to have developed Ozempic face, which is one of the risks of the diabetes drug.
Ozempic could have played a part in Heather Gay's face transformation
Based on side-by-side photos of Real Housewives stars who have sparked Ozempic face rumors, and the subtle loss of volume in the area around Heather Gay's lips, it seems her face transformation could also be partially chalked up to a GLP-1 medication. In a November 2023 interview with People, the "RHOSLC" star confirmed that she had been on Ozempic for quite some time but hadn't seen any significant results. During her November 2025 Bustle interview, she shared that she had eventually switched from Ozempic to Mounjaro.
With the help of the weight-loss drugs, she lost 30 lbs. over three years. In a September 2024 episode of "The Real Housewives of Salt Lake City," Gay confirmed that she was pleased with the results of her transformation, saying, "It's been a long, overdue glow-up. Thank god for modern medicine, that's all I can say." However, she soon developed a new perspective on her transformation. In her December 2024 People interview, she shared that she had found herself reflecting on photos of herself from the early seasons of "RHOSLC" and realized, "I love that girl." She continued, "That girl was braver and stronger and had more confidence being overweight than I did after taking medication."
However, in her Bustle interview, Gay said that she fully intended to use the diabetes drug whenever she saw the number on the scale go up. When all was said and done, Gay's total transformation cost was a whopping $200,000, which included her cosmetic and maintenance procedures and her GLP-1 medications.
