Exclusively for Glam, board-certified plastic surgeon Dr. Raja Mohan explains that Montag is no stranger to going under the knife. "She could have had aesthetic procedures such as rhinoplasty, breast augmentation, facial fillers, Botox, and possibly procedures addressing the jawline," he tells us — and he's certainly spot on, as Montag never hid her plastic surgery journey. She famously decided to undergo 10 plastic surgery procedures in just one day, and she was 23 years old at the time. What many may not even realize is that her 2009 plastic surgery marathon wasn't even her first one, as Montag had already gone under the knife years prior.

Anyone who followed "The Hills" remembers the scene in which Heidi Montag shows her mom everything she had done. "I got a slight eyebrow lift ... I had my nose redone, I had my own fat injected into my cheeks, I had my ears pinned back, I had injections in my lips, I had my chin shaved down," the star explained, as her mother looked visibly shocked. Plastic surgeon Dr. Frank Ryan did all of the work on the star, and he believed these were appropriate decisions for her age. "I disagree that it's too much. Many of these were little tweaks," he told Good Morning America (via U.S. News).
While Montag was entitled to do as she pleased with her body, the truth is that many experts disagreed with her and Dr. Ryan. "It's hard for me to believe that a 23-year-old would need so many procedures," plastic surgeon Dr. Richard Chaffoo shared with U.S. News in 2010. Since then, there's little doubt that Montag stopped tweaking her appearance, as her eyebrows are even more lifted, lips bigger, and overall face snatched with very little movement.
One thing that Montag's plastic surgeries from the late 2000s put into perspective is just how normalized beauty enhancements have become in the entertainment industry. "I just saw some pictures of Heidi Montag from back in the day and was surprised to see how 'normal' she looks for today's standards," a Reddit user pointed out, adding, "She was dragged through the mud by tabloids and labeled as 'fake,' 'obsessed,' and even 'tragic.' Fast forward 15 years, and many of the features she was mocked for are now the gold standard in beauty, especially on social media."

In 2025, Montag explained that in the aughts, there were not many who were vocal about the changes they were making to their bodies and faces. "I definitely was breaking the barrier and the fourth wall of what everyone was secretly doing in Hollywood," she shared (via Independent). "Part of being a trailblazer is when you are the first one to do something, you're usually the most criticized for it. That's something I can accept because I never want to be a liar; that's not the way that I'm built."
In hindsight, Montag agrees that she was too young for such a drastic undertaking, as she admitted to Cosmopolitan in 2019. "I was way too young to make such a life-changing decision and was under so much pressure because it was the beginning of comment sections and negativity and hate on the internet," Montag said (via Page Six). "I was just really self-involved at the time — like so many young people — but I was also on TV where every perceived flaw is amplified."
And we have to give it to her — not many stars were brave enough to open up about their insecurities and the plastic surgery changes they were making. In fact, there are many celebs who confessed to plastic surgery after years of denying it. When they finally admitted to it, it was in a much more acceptable environment, one undeniably created thanks to stars like Montag who always prioritized transparency.
